Annual Return Form for the year ending
December 31, 2011
Click on the
link above which will open the form.
Fill out the form and then click the
third icon from the left at the top of
the form that looks like a diskette.
Save the file to a location on your
computer then email the file to the
Grand Secretaries office.
O.E.S. Supplies Order Form:
You will need a User ID and Password in
order to access the supplies order form.
This User ID and password can be
obtained from the Grand Secretary's
Office. To open the form
requires Adobe reader. If you do
not have Adobe Reader click
here to
download to your computer. Once
you have opened the form click on the Chapter
field. Once you have
filled in the Chapter field you can use
the tab key to advance to the next
fields. The form will calculate the
total in the sub-total field.
Once you have finished filling out the
form, click file, save as or click on
the icon that looks like a floppy disk
to save a copy of the file to your
computer. Attach the file you
have filled out and saved to an email
and email to the Grand Secretary's
Office at
austinb@missourioes.org (The
process of attaching the saved file will be different depending on
which email service or program that you
use. See your email documentation if you
have problems attaching the file.) You
may also print the form that you have
filled out and mail to the address
above.
